Acute phase Rheumatic Fever

Inj. Procain Pencilin 10 lacs x IM x A.T.D daily x 10 (If pencilin sensitive, Tab. Disprin ½ 1 tds x 2-4 weeks (100 mg./kg/day Aspirin)

Complete Bed rest for at least 15 days. If carditis, Bed rest for 4-6 weeks. Tills settled.

If Carditis develops,

Tab. Wysolon 5mg tds x 2 weeks (2mg/kg/day Prednisolone)

If C.C.F. develops,

Tab. LANOXIN 1 tds x 3 then 1 daily x 6 days / week.

Ask for

Hb, WBC, ESR, ASLO Titre, C.Reactive Protein, X-ray Chest, ECG.

For prevention of S.B>E

Inj. Pencom 12 lacs IM x ATD x every month x till the age of 25 years (then for 5 years after the last attack)

Tab. Erythromycin 250mg. OF if sensitive to Pencilin.

Whenever any surgery is done or any infection develops, Give a pencilin cover.

Inj. Pencilin 10lacs. IM x ATD x OD x 3 days.

If a valve lesion develops,

Refer to a cardiologist to evaluate whether it needs any surgery.

Suspect R.H.D. in every chid with fever and Joint pains or chest pain or murmur.

Once Rheumatic fever is diagnosed, the doctor, the relatives and the patient, everyone should know and remember to give pencilin cover for infections and surgical procedures.
